Procedures:

1. Review the online calendar above for availability of items and dates requested.
2. Return downloadable forms to James Wolfe or Mark Carnevalo in Kayser Hall 233 (voice: 554-3028 or 554-3053, fax: 554-2879) or Joyce Sorrels in Kayser Hall 208 (voice: 554-3530).
3. Responsibility of Instructor / Student to fill out the checkout form and pickup and return of the equipment.
4. All equipment comes with tags with equipment list.
5. Priority - first come first serve.
6. Maximum Checkout time - Instructor - 5 days;
   Students with instructor approval - 3 days, recheck out if available.

*** Reminder - disposable products not supplied (example:digital (DV) tapes, DVD's / CD's, etc.)

*** Note - Most equipment was purchased with the intent to use for educational purposes for faculty and students of the College of Education by Technology grant and PT3 grant money. 

What happens if equipment gets lost or damaged?

Lost, stolen or damaged equipment will be reported to proper authorities. Student accounts will be placed in "hold" status if necessary depending on the severity of the loss or damage. This will affect their ability to register for classes, transfer academic credentials, or graduate.
